Most incidents involve human failures as a significant cause, for example; BP Texas City, Esso Longford, BP Grangemouth, Piper Alpha, Bhopal and Buncefield.

Incidents can occur through people’s involvement with their work. As technical systems have become more reliable, the focus has turned to people.

It is estimated, by the Health and Safety Executive, that up to 80% of accidents may be attributed, at least in part, to the actions or omissions of people within a technical system.

Human Factors is now recognised as a significant contributor to organisational reliability, process safety and loss prevention. It is therefore crucial to integrate human capabilities and limitations within complex technical systems. Human factors consideration is a mandatory requirement for offshore safety cases and COMAH safety reports.
